Since 1970, Belington Emergency Squad has proudly served our community, offering medical transportation services for patients going to and from hospitals, physicians offices, skilled nursing facilities and other medical centers as dialysis and radiation treatment facilties. We look forward to helping you schedule your next medical transport.

What we need to schedule transport

Patient Information

  • The patient's name
  • The patient's height and weight
  • The patient's date of birth
  • The time, date and location where the transport will begin;
  • The appointment time and transport destination;
  • The patient's acute medical complaints and special needs;

Medical and insurance information

  • Patient insurance information;
  • Medicare Physician Certification Statement (PCS) for Ambulance Transportation

*Medicare and most insurance providers cover non-emergency ambulance transport when the service is: (1) deemed a "medical necessity," according to certain criteria, and (2) certified by the patient's physician.
