What we need to schedule transport

Patient Information

    • The patient's name

    • The patient's height and weight

    • The patient's date of birth

    • The time, date and location where the transport will begin;

    • The appointment time and transport destination;

    • The patient's acute medical complaints and special needs;

Medical and insurance information

    • Patient insurance information;

    • Medicare Physician Certification Statement (PCS) for Ambulance Transportation

*Medicare and most insurance providers cover non-emergency ambulance transport when the service is: (1) deemed a "medical necessity," according to certain criteria, and (2) certified by the patient's physician.
